Johns Hopkins, founded in 1876, is America's first research university and home to nine world-class academic divisions working together as one university.

Under the supervision of the Clinical Nurse Manager, we are seeking a Sr. Clinical Nurse Case Manager who will provide medical and non-medical case management as part of a multidisciplinary health care and support services team serving people living with HIV and viral hepatitis.

The nurse counsels and manages patients receiving HIV pre-exposure prophylaxis (PrEP) and nonoccupational post-exposure prophylaxis (nPEP), interpreting orders within an RN-led care model. The role also includes testing and treating patients for sexually transmitted infections (STIs), providing prevention interventions in accordance with clinic protocols, and triaging patients in consultation with nurse practitioners, physician assistants, and physicians at the Baltimore City Health Department’s sexual health and wellness clinics.

Specific Duties & Responsibilities

HIV program duties

  • Provide case management for a panel of patients receiving ongoing HIV care, including monitoring and educating patients about their medications, potential side effects, disease processes, and laboratory results.
  • Conduct psychosocial assessments, identify patient needs, facilitate access to recommended services and interventions, and develop individualized care plans.
  • Provide HIV treatment and medication-adherence counseling, including assessing adherence, developing adherence plans, educating patients about HIV treatment and antiretroviral therapy (ART), and preparing pill organizers as needed.
  • Monitor medication adherence and tolerance, and coordinate with pharmacies to confirm prescription fulfillment.
  • Coordinate and administer long-acting injectable HIV therapy, including ensuring appropriate medication storage and handling.

PrEP and nPEP Program Duties

  • Evaluate patients at Baltimore City Health Department (BCHD) Sexual Health Clinics for HIV pre-exposure prophylaxis (PrEP) or nonoccupational post-exposure prophylaxis (nPEP). Order baseline laboratory tests, provide education and counseling, and manage prescriptions under the clinician’s National Provider Identifier (NPI) and in accordance with standing orders.
  • Work with support staff to address financial and insurance barriers to PrEP access and serve as a primary point of contact for patients.
  • Manage a panel of patients receiving PrEP and doxycycline post-exposure prophylaxis (doxy-PEP). Lead regular, site-specific team reviews of the patient panel and coordinate follow-up to support continued use of these services.
  • Coordinate and administer long-acting injectable PrEP, including ensuring appropriate medication storage and handling.

Hepatitis C Program Duties

  • Provide case management for a panel of patients receiving hepatitis C treatment, including monitoring and educating patients about their medications, potential side effects, disease processes, and laboratory results.

General Duties

  • Conduct “express” STI testing visits for patients who require laboratory testing only.
  • Obtain sexual and behavioral risk histories.
  • Educate patients about HIV, sexually transmitted infections (STIs), hepatitis test results, disease transmission, and risk reduction.
  • Collect specimens and coordinate with the laboratory as needed.
  • Assist with point-of-care testing, including rapid HIV and hepatitis C testing.
  • Review test results, communicate results to patients, and refer or triage patients to providers as appropriate.
  • Administer medications and vaccines in accordance with clinic protocols.
  • Assist with prescription refills.
  • Consult with the ordering clinician or medical director regarding complex medical conditions and decisions not covered by standing orders.
  • Triage patients presenting with acute concerns, in person or by phone.
  • Assist with monitoring and restocking clinic medication inventory.
  • Document all care in the medical record in accordance with clinic protocols.
  • Support clinic initiatives, including quality improvement activities.
  • Provide interpretation for Spanish-speaking patients when medically necessary.
  • Complete other administrative and patient-care duties as needed.

Protocol Development and Continuous Quality Improvement

  • Help develop and update case management protocols and standard operating procedures (SOPs).
  • Support the implementation of new protocols, including those for injectable antiretroviral therapy.
  • Oversee medical case management activities and facilitate communication among care teams.
  • Lead continuous quality improvement activities for the HIV continuity program, with a focus on viral suppression, linkage to care, and retention in care.
  • Assist with clinic scheduling and staff-coverage coordination.
  • Coordinate care for patients enrolled in the HIV Directly Observed Therapy program.
  • Complete additional administrative and patient-care duties as needed.

Minimum Qualifications

  • The individual must be a registered nurse, licensed in the State of Maryland.
  • Requires a minimum of two years of related nursing experience, with additional experience preferred.
  • Additional education may substitute for required experience, and additional related experience may substitute for required education beyond an HS Diploma/Graduation Equivalent, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

  • Knowledge of STI/HIV/TB/HCV disease, treatment, prevention, and resources.
  • Ability to work with culturally diverse patients, staff, and community populations.
