Johns Hopkins, founded in 1876, is America's first research university and home to nine world-class academic divisions working together as one university.

The Johns Hopkins Pediatric, Adolescent, and Young Adult HIV/AIDS Program (PAHAP) provides comprehensive HIV medical care and supportive services for infants, children, adolescents, young adults, and families living with or affected by HIV. With Ryan White Part A support, PAHAP provides Health Education and Risk Reduction services through a multidisciplinary model of care.

We are seeking a Clinical Navigator to be a frontline member of the HIV care team who will support patient education, navigation, engagement, and connection to care in a blended role that combines patient education, navigation, engagement (may offer hands on medical assistant support). The position helps patients understand their care, address barriers, strengthen adherence, and remain connected to Ryan White-funded services.

Specific Duties and Responsibilities

Patient Education and Risk Reduction

  • Provide individualized health education related to HIV care, treatment literacy, prevention, risk reduction, adherence, and related health topics.
  • Help patients understand HIV treatment and prevention concepts, including U=U, treatment as prevention, STI prevention, PrEP, PEP/nPEP, and medication adherence strategies.
  • Provide education in a culturally responsive, trauma-informed, youth-centered, and family-centered manner consistent with program goals and role scope.

Navigation, Outreach, and Retention Support

  • Support care plan goals through health education, appointment coordination, referral follow-up, linkage support, adherence education, and patient navigation.
  • Conduct outreach and missed-visit follow-up for patients who are newly diagnosed, transferring care, or at risk of disengagement; identify needs and connect patients with appropriate resources.
  • Assist with re-engagement efforts for patients who are out of care or inconsistently engaged in services.
  • Identify and document barriers to care, facilitate referrals, and elevate unresolved concerns to licensed clinical staff as appropriate.

Clinical Workflow Support, if applicable

  • Support clinical workflows consistent with program protocols, including rooming, vital signs, vaccine support, point-of-care testing, specimen coordination, and clinic flow.
  • Participate in multidisciplinary team meetings, care coordination huddles, case discussions, and quality improvement activities to support continuity of care.
  • Share patient-reported concerns, barriers, adherence challenges, and risk-reduction needs with licensed clinical or Medical Case Management staff for assessment and follow-up.

Documentation and Program Support

  • Document outreach, patient education, referral tracking, encounter follow-up, and care coordination activities in designated systems such as EPIC and CAREWare, as appropriate.
  • Maintain accurate and timely records consistent with program requirements, confidentiality standards, and applicable regulations.
  • Support Ryan White program expectations for linkage documentation, outcome follow-up, and escalation of unresolved barriers.

Scope of Practice

  • The Clinical Navigator does not independently diagnose, certify care plans, provide licensed counseling, or perform duties that require licensed clinical judgment.
  • The position supports care delivery under established program protocols and works in collaboration with licensed clinical, Medical Case Management, and program staff, as appropriate.

Minimum Qualifications

  • Bachelor's Degree in related discipline.
  • Additional education may substitute for required experience and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

  • Completion of an accredited Medical Assistant, Health Education, Community Health Worker, Patient Navigation, Public Health, or related training program.
  • Current CPR/BLS certification strongly preferred or willing to obtain within 6 months of employment.
  • Experience in primary care, adolescent medicine, infectious disease, public health, health education, patient navigation, or HIV care.
  • Experience working with people living with HIV, youth and young adults, families affected by HIV, medically complex individuals, or underserved populations.
  • Experience providing health education, risk-reduction support, adherence support, outreach, linkage, retention-in-care, or patient navigation services.
  • Familiarity with Ryan White HIV/AIDS Program services, HIV quality measures, Baltimore EMA service expectations, and community-based HIV resources.
  • Experience using EPIC, CAREWare, or other healthcare documentation and reporting systems.
